Glucose Dependent Insulinotropic Polypeptides
Glucose-dependent insulinotropic polypeptide, or GIP for short, is an inhibitory hormone. It functions as an inhibitor of the secretin family of hormones. Its main role is to stimulate our insulin secretion. By stimulating the insulin secretion GIP is known to be a regulator of our blood sugar along with glucagon-like peptide 1 (GLP-1).
Glucagon-like peptide 1 is a peptide with up to 31 amino acids that is considered to be a peptide hormone. It derives from tissue specific post translational processing of the peptide proglucagon. It is often found in the intestinal enteroendocrine L-cells as this is where it is both produced and secreted from.
Both GLP-1 and GIP are part of the incretin metabolic hormones. These hormones work to decrease blood glucose levels and are released after the consumption of food. It is important to regulate these groups as they have the ability to aid in both blood glucose levels as well as potential weight loss.
What is Tirzepatide?
Tirzepatide is a peptide with the ability to assist with glycemic control. Eli Lilly and Company applied for its first patent in early 2016 with its approval occurring later that same year. It is primarily sold as a type of type 2 diabetes medication. However, it has also found a home as a treatment for weight loss for obese and overweight patients. In early 2018, Eli Lilly and Company announced these findings after the completion of the SURPASS-2 trials.
Tirzepatide Molecular Structure
Tirzepatide is an analog of the GIP hormone. The largest difference is that it has a C20 fatty di-acid attached to it. This di-acid is used as a way to optimize the uptake and metabolism of tirzepatide. The overall construction of tirzepatide allows for a longer half life which would mean the extension of down time between necessary doses.
Obesity & Overweight Treatment
A study which included obese mice tested tirzepatide’s ability to aid in weight loss as well as insulin sensitivity. In this study researchers found that tirzepatide improved insulin sensitivity in weight dependent as well as weight independent manners. The obese insulin-resistant mice in the study were each given a dose daily for 14 days. The dose for tirzepatide was 10 nmol/kg. Several tests checking various levels including tissue weight, plasma levels, leptin triglycerides and free fatty acids among other tests were taken. This was to surmise tirzepatide’s benefits in its subjects.
Tirzepatide Benefits
Tirzepatide is a dual glucagon like peptide 1 and glucose dependent insulinotropic peptide receptor agonist. It has the ability to significantly reduce glycemic levels as well as improve insulin sensitivity. Not only that, tirzepatide is also well known for its ability to reduce body weight by more than 20% while improving lipid metabolism.
- Reduce Glycemic Levels
- Improve Insulin Sensitivity
- Reduce Body Weight
- Improve Lipid Metabolism
Tirzepatide Side Effects
While tirzepatide is a peptide, it can create some unwanted side effects. These side effects include things like nausea and vomiting, diarrhea or constipation, and possible abdominal pain or discomfort. It should also be noted that tirzepatide should not be used by those who have a family history of medullary thyroid cancer or who have multiple endocrine neoplasia syndrome type 2.
- Nausea
- Vomiting
- Diarrhea
- Constipation
- Possible Abdominal Pain
Tirzepatide Dosing Protocol
Common tirzepatide doses vary from fairly low to the highest dose being 15mg. Things to keep in mind, the higher the dose, the higher the probability of side effects. That being said, the lowest dose usable for tirzepatide would be 2.5mg/0.5mL. Once you know how your body reacts to the peptide you can begin the process of increasing the dose if necessary. However, it is not recommended to exceed the top dose of 15mg/0.5mL.
A Common Dosing Protocol Used for Type 2 Diabetes Patients
First Dose: 2.5mg once a week.
After 4 Weeks: 5mg once a week.
Anything after the four weeks should only be increased by 2.5mg at a time if needed for tirzepatide. Note, these doses have also been used for those looking to lose weight as well and can be adapted for these differences as needed.
